Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Dental Insurance
All financial obligations are scheduled prior to treatment commencing. We are in network with Presbyterian Medicare Advantage through DentaQuest and Aetna DMO insurance and accept patients with other dental insurances as out-of-network provider.
Upon patient request, we will file a pre-authorization with your insurance carrier to determine how much your insurance will cover for your treatments.
Financial Policy
We can help you maximize your benefits. We recommend that you contact our office prior to your appointment to provide us with insurance information for verification of benefits. We accept most major dental insurance and will file most claims for you as a courtesy. We will provide you with an explanation of your dental benefits and financial responsibilities up front, and work with you to ensure financing if needed.
